The primary responsibility of this position is to perform intensive case management services to assess the needs, strengths, and barriers of Food Share Employment and Training (FSET) Program participants. Work collaboratively with participants to develop a plan that works to assist them in reaching their education and employment goal(s). SUPERVISES No supervisory responsibility. RESPONSIBILITIES A. ESSENTIAL FUNCTIONS 1. Provide case management services including locating, managing, coordinating and monitoring all services and informal community supports needed by FSET participants. Services include assessment, case planning, monitoring, review, advocacy and referral. 2. Case management activities provided through the team approach are: a. Summary and analysis of subjective and objective data through individualized assessment. b. Planning of appropriate activities to address the assessment results. c. Determining the intervention strategies that will address the identified needs and goals. d. Implementation of the plan and intervention strategies. e. Ongoing progress monitoring and evaluation of the individual\'s identified goals and employment plan. 3. Ability to motivate others towards achieving their personal goals. 4. Ability to work independently with strong sense of focus, task-oriented, nonjudgmental, open personal qualities, clear sense of boundaries. 5. Ability to work in a variety of settings with culturally-diverse families and communities with the ability to be culturally sensitive and appropriate. 6. Provide holistic and comprehensive case management services to all participants. Including: intake assessment, benefit assessment, goal setting, long and short-term case plan development, progress monitoring, advocacy and referral. 7. Excellent written and verbal communication skills, ability to establish rapport with participants and Income Maintenance staff. 8. Promote a strong sense of and respect for confidentiality, involving both clients and fellow employees. 9. Develop relationships with local employers to understand in-demand jobs and assist program participants in gaining employment. 10. Assist participants in completing job applications, writing a resume and documenting work history. 11. Develop workfare site and work experience sites, monitoring participation in both programs. Please visit https://careers.co.wood.wi.us/ for complete list of duties. **** * OTHER EXPERIENCE AND QUALIFICATIONS: *Knowledge of human behavior, motivation, crisis intervention, family and group dynamics, money management, and organizational skills are necessary. *Interviewing and assessment skills are required. *Requires working independently with a strong sense of focus, while being task-oriented, and possessing non-judgmental, open personal qualities, and a clear sense of boundaries. *Basic everyday living skills are needed, as is the ability to understand and follow oral and written directions. *Must possess excellent written and verbal communication skills; ability to establish rapport with participants and Income Maintenance staff. *Reading, adding, and subtracting skills are needed. Must have access to transportation to travel to various work sites.
