Winona Health Community Health Worker in winona, Minnesota

Description 0.8-1.0 FTE, 64-80 hours bi-weekly. Variable shifts between 7 am and 6 pm. Weekends as needed to meet the needs of patients. Winona Health is a registered Community Health Worker Apprenticeship site through the Minnesota Department of Labor and Industry. This may be an ideal opportunity for those interested in a Community Health Worker Training Program (CHWTP) Apprenticeship. CHWTP Apprenticeship participants may be eligible to receive an additional short-term stipend in addition to wages. Position Overview: The Community Health Worker employs community-based care coordination strategies outlined in the Pathways Community HUB model to improve health outcomes for targeted populations. Through community-based care coordination this position will provide support to at-risk clients as they navigate the health care and social service systems in the Winona area. This position aims to increase access to support services, improve the quality and cultural competence of their care, and increase the health knowledge and self-management skills of clients served. The Community Health Worker has a deep knowledge and understanding of the Winona-area community, and holds lived experience that uniquely equips them to bridge barriers related to culture, race, language, socioeconomic status or other factors. This position requires extensive travelling throughout the community working with varied populations, oftentimes in client homes. This position works in partnership with the Live Well Winona (LWW) Winona Community HUB and works closely with Winona Health staff, area social service organizations and other partners. *Position may be partially or fully funded by grant(s).
