Job Summary

To provide Junior Faculty advanced training in low-risk and high-risk obstetrics and newborn care to Family Medicine Practitioners who will work in underserved communities, or as faculty.

Essential Functions

  • GOALS:1. Manage simple and complex labors.2. Attain competency in operative obstetrics, including Cesarean section, instrumented deliveries, tubal ligation, and amniocentesis for assessment of fetal lung maturity.3. Provide competent prenatal care to pregnancies complicated by diabetes, hypertension, twin gestation, thyroid disease and mental health disorders.4. Stabilize and treat pregnant patients hospitalized for pre-term labor, pre-eclampsia, diabetes Class A, and placental disorders.5. Perform biophysical profile and limited obstetric ultrasound.6. Counsel obstetric patients regarding genetic risk and know the appropriate patient and timing to refer for CVS vs. Amniocentesis.7. Resuscitate distressed newborns.8. Provide ongoing care to the hospitalized newborn.9. Maintain Family Medicine clinical skills.10. Develop and teach competency in obstetrics to Family Medicine residents and family practitioners.

  • OBJECTIVES:1. Evaluate obstetric patients and their fetal monitoring strips in triage.2. Provide labor management, vaginal delivery and instrumented deliveries to obstetric patients.3. Perform Cesarean sections as primary surgeon. Surgical skill level at the beginning of the fellowship will determine when the fellow is ready to be the primary surgeon.4. Perform post-partum tubal ligation.5. Perform urgent/emergent D&C’s on emergency room and post-partum patients.6. Evaluate and co-manage ectopic pregnancies from the emergency room.7. Supervise admissions and daily management of antepartum service patients.8. Perform amniocentesis for fetal lung maturity on the antepartum service.9. Provide initial consultation and ongoing prenatal care to patients. Focus will be on patients referred from Family Medicine clinics for difficult pregnancy.10. Perform limited obstetric ultrasound at the Family Medicine center, i.e. Biophysical profile, dating, placenta placement, position.11. Perform newborn resuscitations under the supervision of Neonatal Nurse Practitioner and/or Neonatologist. Resuscitations will include intubations and umbilical vein catheterization.12. Admit and manage newborns in step down and/or special care nursery.13. Continue to see continuity patients 1-4 half-days a week.   • I. LABOR AND DELIVERY ROTATION (6 Months):A. GOALS:1. Manage simple and complex labors.2. Attain competency for operative obstetrics, including Cesarean section, instrumental deliveries and tubal ligation.3. Provide post-operative care for such patients.4. Develop and maintain competency in teaching obstetrics to Family Medicine residents.5. Resuscitate distressed newborns.6. Provide ongoing are to the hospitalized newborn.B. OBJECTIVES:1. Evaluate obstetric patients and their fetal monitoring strips in triage.2. Proved labor management, vaginal and instrumented deliveries.3. Perform Cesarean sections as primary surgeon with obstetrical physicians.4. Teach 1st year residents the OB service basic obstetrics, cervical exams, labor management, Pitocin augmentation, IUPC, FECG, vaginal delivery and post-partum care.5. Perform post-partum tubal ligations.6. Perform neonatal circumcisions.7. Perform urgent/emergent D & C on ER and post-partum patients.8. Evaluate and co-manage ectopic pregnancies.9. Perform newborn resuscitation under the supervision of the Neonatologist (or

  • II. ULTRASOUND ROTATION (1 Month):A. GOALS:1. Perform ultrasound during pregnancy.2. Become competent in performing biophysical profiles, AFI, EFW, fetal position, placenta placement and EDC.3. Use ultrasound skills in the Family Medicine office, L & D and Antepartum ward.4. Teach ultrasound skills to Family Medicine residents and faculty.B. OBJECTIVES:1. Perform ultrasound on obstetric patients when needed.2. Supervise Family Medicine residents in performing ultrasounds.3. Perform amniocentesis for fetal lung maturity when required.4. Provide initial consultation and ongoing prenatal care to patients. Focus will be on patients referred from Family Medicine residents. Units may be in consultation with perinatologist.

  • III. GENETICS ROTATION (1Month):A. GOALS:1. Attain competency in identifying which patients need formal consultation with MFM and/or Genetics.2. Counsel patients appropriately on CVS vs. amniocentesis, i.e, which patients are better suited to which procedure.3. Appropriately counsel patients in the office setting concerning quad screening and first trimester screening. Accurately counsel patients on how to proceed with abnormal results.B. OBJECTIVES:1. Evaluate obstetric patients through consult and discussion.2. To be able to identify which patients need formal consultation.3. To be able to discuss each procedure, pros, cons and percentage of risks.

  • IV. NEONATAL ICU ROTATION (1 Month):A. GOALS:1. Resuscitate distressed newborns.2. Provide ongoing care to the hospitalized newborn.B. OBJECTIVES:1. Perform newborn resuscitations under the supervision of the Neonatologist, including intubations and umbilical vein catheterizations.2. Continue management of newborns in special care nursery.3. Perform complete resuscitation and newborn care as part of the neonatal resuscitation team.

  • V. ELECTIVE ROTATION (1 Month):A. GOALS:1. To attain increased comfort and knowledge in OB and/or Family Medicine.2. To obtain additional training suited to individual clinical practice.B. OBJECTIVES:1. Attain competence in specific procedural or cognitive skills, which may include increased competence in colposcopy/LEEP, ultrasound, Family Medicine clinic, more advanced surgery.2. To have continuity clinics continue through the month.

  • VI. GYNECOLOGY/ONCLOGY ROTATION (1 Month):A. GOALS:1. Develop increased skill at recognizing abnormal patterns with colposcopy. Understand and be knowledgeable on the different cervical conization procedures.2. Know the indications for cone and be able to counsel patients.3. Competently perform LEEP procedure.4. Recognize and manage complications from LEEP.5. Know indications for vulvar biopsy and treatment.6. Determine when and if endometrial sampling is necessary and be competent in the procedure (D&C vs. Endometrial sampling).B. OBJECTIVES:1. To attain competence in specific procedures.2. Supervise and teach fellow Family Medicine residents indications and procedures.a. I.e.: LEEP, endometrial biopsy, vulvar biopsy, D & C and Hysteroscopy, as applicable.3. Scrub in as 2nd or 3rd assistant in major Gyn-Onc surgery. To become familiar with the procedures.a. I.e.: Length of procedure, estimate blood loss, risks involved and post-op care.4. Counsel patients about procedure and follow-up care, as applicable.

  • VII. PERINATOLOGY ROTATION (1 Month):A. GOALS:1. Provide competent prenatal care to pregnancies complicated by diabetes, hypertension, twin gestation, thyroid disease, cardiac disease, renal disease and mental health disorders2. Stabilize and treat pregnant patients hospitalized for pre-term labor, pre-eclampsia diabetes and placental disorders.3. Perform biophysical profile and limited obstetric ultrasound.4. Counsel patients regarding genetic risks.B. OBJECTIVES:1. Supervise Family Medicine OB admissions and daily management.2. Perform amniocentesis for fetal lung maturity.3. Provide initial consultation and ongoing prenatal care to patients. Focus will be on patients referred from Family Medicine residents.4. Perform limited obstetric ultrasound and biophysical profiles.5. Co-counsel patients referred to genetics counselors.6. See patients with perinatologist in MFM clinic.

  • OTHER GOALS:I. CONTINUITY CLINIC:1. Maintain knowledge and skills as Family Practitioner.2. Build a practice suitable for billing.3. Perform prenatal care for their private patients and be responsible for their care in L & D.4. See patients on the same schedule as attending physicians.II. PRECEPTING:1. Maintain knowledge and skills as Family Practitioner by working with residents.2. To be able to teach fellow Family Medicine resident procedures and indications, incorporating their new knowledge in Ob and Gyn procedures along with Procedures normally don in Family Medicine office.III. TEACHING:1. Give two OB and Gyn lectures at the Wednesday morning Family Medicine lectures.2. Give some of the lectures and presentations for the medical students and OB/Gyn residents while on the L&D rotation.3. Present interesting OB/Gyn cases at the Family Medicine case conference on the last Thursday of each month.
