SUMMARY Under general supervision, this Ambulatory Nurse Case Manager for pediatric departments, coordinates and manages all aspects of patient care through multiple care delivery systems, acting as the central point of contact for patients, supporting them through transitions of care. Receives case management reports and primary care physician (PCP) referrals to target very high and high-risk patients to ensure quality care and patient safety. Through initial contact and assessment, establishes active partnerships with patients and clinical partnerships with PCPs, specialists, primary care leadership and staff, and internal and external resources.
EDUCATION/LICENSES/CERTIFICATIONS Graduate of a state-approved school of nursing, Baccalaureate degree preferred (or equivalent education, training or experience). Certified Case Management (CCM) or CCM eligible strongly preferred. Must possess a current unrestricted RN licensure in the Commonwealth of Massachusetts.American Heart Association Basic Life Support (BLS) required. Advanced Cardiac Life Support (ACLS) may be required based on specialty.
EXPERIENCE Minimum of 3 to 5 years nursing experience in case management/continuing care in an inpatient or community setting strongly preferred; exceptions in consultation with nursing leadership. Experience with an electronic medical records system, EPIC experienced preferred.
SKILLS Must exhibit strong critical thinking, problem solving, interpersonal and good clinical assessment skills. Highly motivated and self-directed. Ability to interpret clinical information, assess the implication of treatment and develop and implement a plan of care. Demonstrated ability to prioritize, multitask, and work in a rapidly changing environment with multiple demands. Ability to work effectively in a team. Strong Computer Skills
