Life and Community Advocators perform a wide variety of person centered supports and services with a strong emphasis on personal choice to individuals with developmental disabilities. These supports and services involve advocating for, encouraging, guiding and teaching individuals in expressing personal choice, developing daily living skills and habits, taking care of their personal needs, assuring community integration, ensuring for their health and safety, and maintaining a healthy, safe home environment. The supports and services performed depend upon the specific abilities of the individuals being served and the nature of the setting where the activities are performed.

[[Position Summary: [[The incumbent is responsible for supporting individuals with developmental disabilities in a community-based home. You will provide assistance to the individuals in the development of linkages to the community through recreation planning, transportation, accompaniment to community-based events, and in personal care as needed. Other responsibilities include providing assistance with household maintenance, budgetary/financial management, shopping, and meal preparation. • Implements Nursing Plans and follows-up on nursing recommendations.
• Handles medical appointments as assigned. Arrives for appointments on time, with appropriate information about the individuals medical history and current condition.
• Administers medications according to regulations.
• Observes and documents medical issues in a clear and timely manner, providing follow-up as required.

• Reviews plans and demonstrates familiarity with each individuals plan
• Implements each individuals plan as prescribed and asks for clarity if necessary.
• Records data on outcomes and Target Behaviors accurately and promptly.
• Demonstrates familiarity with Behavior Support Plans and implements plan in a consistent manner.
