Job Overview:
A pharmacy technician will participate in the medication reconciliation process that will include accessing the electronic medical record EMR in the Emergency Department ED and identify patients who have been in the ED for a specified time period with a note in the chart indicating they will be admitted.
Qualifications:
Required:
High School diploma or equivalent
NJ Pharm Tech Registration
Preferred:
Associates degree or Bachelor of Science
1 year previous pharmacy experience
National CPhT certification
Scheduling Requirements:
Evening Shift
40 hours per week
Weekend/holiday requirements
Essential Functions:
This pharmacy technician assists in the provision of pharmaceutical services under the direct supervision of the registered pharmacist by performing the technical, clerical and manipulative tasks
Work and/or performance shall conform to written established hospital policies and procedures
Function of Medication Reconciliation Technician: A pharmacy technician will participate in the medication reconciliation process that will include accessing the electronic medical record EMR in the Emergency Department ED and identify patients who have been in the ED for a specified time period with a note in the chart indicating they will be admitted
The technician will interview the patient or their family members to obtain an accurate medication history
The technician will also need to review the currently documented external prescription history and contact the patient's retail pharmacy to resolve any medication concerns
In addition, they will need to update and reconcile the patient's home medication list in the EMR
Good communications skills and customer service are required for interviewing patients and their families, as well as relaying the use of non-formulary medications to the care team
Having a working knowledge of healthcare informatics is vital for the entry and documentation of their daily activities in the pharmacy documentation database
Performs other duties as required by the department
