Shift : Days - Mon-Thurs - 8:30a-5p
The community you’ll be caring for: AdventHealth Waterman Community Clinic – 2300 Kurt Street, Eustis, FL 32726
The role you’ll contribute: The Care Management Assistant (CMA) provides case review of clinic patients and resources and is responsible for coordinating communication between patients and available resources. The CMA has a general working knowledge of community partners and community and state programs. The CMA assists in compiling data related to patient outcomes and auditing data for accuracy.

· Meet with patients and/or families to discuss realistic and appropriate referral options as part of the eligibility process.
· Assist patients with eligibility process completion to have them established with the clinic.
· Considers unique physical, psychosocial, and financial needs of the patient, and anticipates and/or identifies barriers while completing patient eligibility process and/or visit check out.
· Demonstrates and encourages positive interpersonal relations with the patient/family, physicians, community agencies, and other members of the health care team. Maintains open communication.
· Effectively utilizes internal/external resources for referral assessment and planning.
· Utilizes EMR system for patient appointments, and scans documentation on a daily/weekly basis.
· Completes Prescription Assistance Program Applications/refill forms as needed.
· Greets patients and visitors upon arrival and offers assistance as needed.
· Answers phones and demonstrates positive communication with all.
· Maintains inventory and department logs.
· Retrieves and distributes mail.
· Maintains orderly work environment, and confidentiality.
· Manages time and resources effectively to assure required tasks are completed in the most efficient, cost-effective manner.
· Adapts quickly to changes in patient status and update assessment and referral plan as patient’s situation warrants.
· Supports Director/Manager/Supervisor with special projects and maintaining department statistics
· Supports discharge care managers in directing referrals, communicating patient information, and serving as liaison with physicians, hospital staff, and community agencies/liaisons.
· Compile reports for statistical data and reporting functions
· Promote the optimal well-being of vulnerable, oppressed, and marginalized populations by building on their strengths and resilience through collaborative community partnerships.
· Receive/direct telephone communications with care management department, and patients.
· Provide administrative support for department supervisor, manager and director, coordinating calendar/meetings, prepares letters/other communications.
· Serves as a liaison/representative of Care Management to other Hospital Departments.
· Other duties as assigned/requested by the department supervisor/manager/director.
· Knowledge/experience with medical terminology.
· Position requires excellent assessment, critical thinking, and problem solving and effective verbal/written communications.
· Exhibits organizational skills, flexibility, and teamwork
· Excellent customer service and telephone/communication skills.
· Ability to function in a stressful and fast paced environment.
· Excellent computer skills with proficiency with MS-Office Suite and hospital-wide electronic systems.
· Efficient time management, ability to maintain organized work environment and facilitate work flow.
· Required to sit and/or stand for long periods of time.
· Provides necessary coverage and assistance with other duties within the department when needed.
· Ability to flex work hours when necessary.
Qualifications
The expertise and experiences you’ll need to succeed:
REQUIRED:
· Minimum of a High School Diploma or Equivalent
PREFERRED:
· Associate’s degree in a health-related field or equivalent
· Previous hospital care management experience preferred.
· Clinic/medical office experience.
