Shift Details: Full time - 40 hours/week
Schedule: 8:00am - 4:00pm, Monday through Friday (extended hours as needed)
Compensation: $55,000 annual salary

About the job:
Performs skilled nursing functions in accordance with federal and state laws. All functions are performed in accordance with the established policies and practices and the State Nurse Practice Act. Delivers care to program participants/residents as delegated by a Registered Nurse (RN) or a physician.

Essentials:
Under the auspice of an RN, through observation, determine the resident's nursing needs to be included in their plan of care
Monitor and document resident's progress. Under the auspice of the RN, provide needed health checks
Administer medications prescribed by physician. Assist in Medication Administration Certification Training
Perform non-invasive medical procedures such as dressing wounds, injections, etc.
Perform any emergency procedures for residents
Assist in coordinating and communicating resident care with care team, outside providers and agencies, family members
Provide health education including self-care and wellness to residents and their families
Provide necessary training and education to staff and clinicians. Serve as an advocate for residents
Assist in maintaining the current health and medical record of each resident
Inform appropriate management of pertinent medical findings and recommendations for follow-up with outside agencies
Schedule routine physicals and medical, dental examinations, as needed
Promote personal safety and a safe environment for participants and coworkers. Responsible for ordering needed medical supplies
Assure that program participant privacy is maintained by being knowledgeable and by following HIPAA privacy and security regulations
Required Qualifications:
Graduated from an accredited school of nursing with and maintaining a current MN LPN license.
Preferred two years' experience and, behavioral health experience preferred.
Experience using Electronic Health Records
Communication, teamwork, problem-solving, time management and punctuality.
