Intermountain Health Pharmacy Director-HC in South Jordan, Utah

Job Description:

This position serves as the key leader responsible for all pharmacy services and medication management for home health pharmacy services at Intermountain. The director strives to meet all established outcomes for clinical excellence, operational effectiveness, customer service, provider engagement, caregiver engagement, and leadership. This leader will ensure that Pharmacy Services delivers extraordinary care through implementing clinical, operational, and service initiatives. The pharmacy director works with local and enterprise facility leaders to ensure patient needs are being met. The director supports the enterprise pharmacy director through coordinating with the facility operation leader and hospital pharmacy directors when required. This position leads the day-to-day pharmacy operations and ensure safety of patients while engaging with the strategic direction of the facility. The pharmacy director establishes interdisciplinary relationships to ensure the quality of all medication therapy provided. This position is responsible for human resource responsibilities of the department. The director is responsible to participate in both Facility and Enterprise initiatives and goal planning. Additionally, this role may be asked to lead initiatives a as part of the extended pharmacy services team and any other responsibilities relevant to this position.

Scope

As part of Shared Services, this position has direct accountability to the system pharmacy operations director assigned to Home Health and a "dotted-line" reporting relationship to Home Health leadership.

Job Essentials

Leadership:

Oversees and participates in daily activities required of staff. Is responsible for creating a culture of accountability in the delivery of effective and efficient pharmacy services. Provides leadership consistent with Intermountain's Leadership Competencies. Is ultimately responsible for oversight of all aspects of medication management, including acquisition, prescriber ordering, preparation, dispensing, administration, clinical management, monitoring, and appropriate disposal. Serves on committees locally and at the system level as a leader in medication management policies and processes.

Operational Effectiveness:

Creates and sustains a culture of continuous improvement in quality, safety, and operations. Is responsible to effectively manage both capital and operational budgets, and financial stewardship of the medication use process for Home Health Pharmacy. Takes a leadership role in local and system business development strategies. Ensures compliance with all applicable laws, regulations, and accepted best practices.

Clinical Excellence:

Creates and sustains culture of continuous clinical improvement. Consistently incorporates local, system, and accepted national clinical best practices and medication safety best practices into operations. Collaborates closely with Quality and Patient Safety, to assure processes and care offered are continually evaluated and improved.

Employee Engagement:

Creates a culture of engagement at the group and individual level. Leads staff and pharmacy leadership in identifying, developing, and implementing action plans that improve engagement. Communicates structure, goals, initiatives, activities, successes, and failures to employees. Ensures professional development is organized, provided, and achieves competence for identified educational strategies with appropriate staff. Actively promotes participation in professional development.

Patient Engagement:

Is responsible for provision of excellence service to all internal or external customers. Regularly participates in rounding. Inspires staff members to improve service in all areas of the patient/customer experience. Identifies and addresses all deficiencies by developing and implementing improvement plans. Is accountable to work closely with providers and caregivers to improve patient service scores.

Physician Engagement:

Supports Pharmacy and Therapeutics (P &T) processes with guidance from Enterprise P&T, and engages physicians to support formulary and medication use initiatives. Ensures engagement with system P&T with pertinent pharmacy and physician leaders. Partners with physicians to provide care and optimize patient care provided. Ensures compliance with various medical staff related regulations.

Community Stewardship:

Participates in community outreach activities. Supports staff participation in community service and professional organizations. Supports student and residency programs.

Minimum Qualifications

Pharm D or Bachelor of Science in Pharmacy from an accredited institution (degree will be verified).

Master's degree in one of the following fields: MBA, MPH, MHA, or MS in pharmacy from an accredited institution (degree will be verified), or or 10 years pharmacy leadership experience in a comparable setting.

State license to practice pharmacy and dispense controlled substances.

5 years of hospital pharmacy leadership experience at a facility of comparable size and complexity, preferably in a Homecare setting.

Preferred Qualifications

Pharmacy residency, other post-graduate training, and/or board certification.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

$76.21 - $109.81
