Job Summary

The Hospice Registered Nurse Liaison is imbedded at Robert Wood Johnson University Hospital and works in a team of Grace Healthcare professionals consisting of other nurse clinical liaisons, a Social Worker, and a Chaplain, all exclusively focused on providing hospice care and end-of-life information to RWJ patients, families, staff and providers.

You will be a fierce patient and hospice advocate and provide the link between Grace hospice services, terminal patients in/affiliated with Robert Wood Johnson University Hospital, and RWJ physicians, Case Managers, Social Workers, and Palliative medicine practitioners. You will accomplish this by using your marketing insight, providing exemplary customer service, relationship-building skills, end-of-life knowledge, and clinical expertise.

    Job Description:

  • Promptly respond to all hospice referrals by evaluating the patient for hospice eligibility and the correct level of care, whether as General In-Patient or routine level of services in the community and continue to follow-up/assess in-patients as needed, all while providing excellent customer service.

  • Provide info/education about hospice services/philosophy, individualized disease/illness trajectory, and EOL expectations to the patient, pt representatives, and hospital care teams.

  • Communicate and collaborate with inpatient and outpatient teams, providers, facilities, patient and pt representatives to maintain continuity of care, identify patient needs/goals of care, establish/facilitate safe discharge planning/needs and manage symptoms at EOL

  • Use technology and other resources to ensure necessary, compliant, and detailed documentation for Medicare/Medicaid, hospital, and Grace Healthcare need.

  • Serve as a clinical resource for facility staff, physicians/APNs, patients, and families for symptom management recommendations, appropriate outpatient or inpatient placement, goals of care setting, and leading/participating in challenging EOL discussions.

    Competencies and Skills:

  • Strong knowledge of government hospice regulations, Medicare requirements, and POC development required (for GIP and routine level of care)

  • Understanding of hospice philosophy, principles of EOL care, and hospice care for inpatient/outpatient

  • Advanced critical thinking and problem-solving skills

  • Ability to work as part of a tight collaborative team internally/externally and form trusting harmonious working  relationships.

  • Confident self-starter with a high degree of initiative, flexibility, motivation, energy, creativity, and time management skills who is assertive, diplomatic, and a patient  advocate.

  • Strong clinical knowledge of disease states, prognosis and progression, associated disease symptoms, and medications/doses to recommend for comfort

  • Comfortable with educating and training patients, families, hospital staff, and providers on an ongoing basis about hospice, comfort care, EOL, and symptom management  

    Experience and Education:

  • Valid NJ nursing license; BSN preferred

  • CHPN desired

  • Minimum recent 3 years inpatient/acute nursing experience required

  • Plus 2 years of clinical hospice experience

  • Knowledge of dc planning or case management preferred
