MEDICAL PHYSICIAN (Independent Contractor) – Craiglee Nursing Home

We are currently posting the above position of Medical Physician for this 169 bed Long Term Care Home, located in Scarborough, Ontario. This Home features a supportive team of nursing, dietary, activation and administrative staff with a holistic approach to individualized resident care in a clean and well-maintained environment

 RESPONSIBILITIES

  • Attends the nursing home on a weekly basis, to provide clinical care to assigned residents.

  • Addresses resident acute and chronic care needs and provides appropriate and timely documentation.

  • Identifies circumstances in which palliative and/or end of life care may benefit the resident and family members and work collaboratively with members of the interdisciplinary team.

  • Works effectively and efficiently with members of the interdisciplinary team including the medical director to ensure the highest level of care is provided to residents.

  • Conducts regular medication drug review and documents/updates resident’s health record.

  • Participates in after hour on-call.

  • Commits to continuing education related to long term care, care of the elderly, palliative care, and dementia care.

  • Participates in physician peer review to continually improve resident care.

  • Direct and organize the Facility's Physician’s services as required by applicable legislation and Facility policies, including but not limited to the areas of medical service provision, administration, and education.

  • Provide advice and be accountable for the areas of administrative medical services and policies in the Facility, including: o development, implementation and evaluation of medical services, o annual review of the medical policies o preparation of an annual report summarizing the strengths and areas of concern within the medical services program, o quality assurance/improvement activities and processes, o involvement in Facility-wide issues, such as infectious disease outbreak management, and o participation in the process to develop the mission, philosophy, goals and objectives of the Facility

  • Provide advice to the Board of Directors/Owners/Administrator of the Facility on matters pertaining to medical care and services.

  • Actively participate as a member of the following Facility committees: (i) Infection Control (ii) Professional Advisory (iii) Health Records

  • Participate and/or to attend such other committees as mutually agreed upon between the Medical Director and the Executive Director.

  • Commit to continuing medical education and maintaining current knowledge and practice in geriatrics and gerontology

  • Visit the Facility regularly, at least once per week in order to carry out his or her responsibilities under this agreement.

  • Complete all forms pertaining to the medical services program as required.

  • Advise the Executive Director, in co-operation with the Medical Officer of Health, of the steps to be taken in the event of an outbreak of communicable disease.

  • Participate in the review of Placement Co-ordination Service applications.

  • Assist or participate in resolving resident issues that may put other residents, staff, or visitors at risk

  • Assist in staff development as required and as jointly negotiated with the Executive Director or his or her designate

  • Participate in in-service education programs each year

  • Provide medical liaison with local hospital(s) and other appropriate health care organizations or facilities.

  • Sign an Attending Physician's Agreement

  • Provide such other services in relation to the provision of medical services in the Facility that are reasonably required, and which are jointly agreed upon by the Medical Director and the Executive Director.

  • Advise the Facility on emergency medical services for residents

  • Participate in care conferences for residents QUALIFICATIONS

  • Medical practitioner license to practice medicine in the Province of Ontario, and   always be a member in good standing of the Colleges of Physicians and Surgeons of Ontario.

  • Experience working in Long Term Care preferred.

  • Knowledge of regulations governing long term care.

  • Ability to work effectively as part of an interdisciplinary team.

  • Ability to build trust and interact in a professional manner with staff, residents, families, and other community partners.

     The Home’s administration provides a monthly stipend, enhanced remuneration package and assists with providing the Health Card Numbers and other administrative support.

    All Applicants must be currently registered in good standing with the Ontario College of Physicians and Surgeons.
