Country Manor's Health Care & Rehab Center is now hiring an RN Case Manager to join their award-winning team!

Responsibilities as an RN Case Manager:
Provide resident care and treatments
Ability and desire to lead and inspire a team of nursing staff
Comfortable with delegating tasks
Collaborate with IDT
Educate and update residents on medical treatments
Qualifications:
Current Registered Nurse license in the state of MN
CPR certified
Previous experience in nursing or other medical fields
Previous Case Management experience preferred
Familiarity with medical software and equipment
Ability to build rapport with staff, residents, and families
Strong communication and leadership skills
Ability to thrive in a fast-paced environment
