Salary: $75,000.00 $85,000.00 per year, $2000 hiring bonus ($1000 at 30 days of hire and $1000 at 6 months of hire) This is a full time position. Duties As a Licensed Care Professional, you will work across Wilder s Customized Living Services provided at two Saint Paul Public Housing sites (Ravoux and Hamline Hi-Rises). Together with Wilder s Healthy Aging & Caregiving Services, you will join a collaborative team that supports the health and independence of adults to age and be cared for in the community, critical to Wilder s mission of improving lives today and for generations to come! In your role, you will provide professional nursing services on a person-centered basis to older adults and adults with chronic conditions. You will provide supervision and work direction to licensed health care staff and training for health care technicians, assist providers and others to manage healthcare needs, assess clients, and monitor medication needs of clients. Communicating regularly with an interdisciplinary team and completing documentation as directed is also essential.
