On-call rotation after training. ATTRIBUTESCompletion of residency OR fellowship training OR board certification through the Board of Pharmaceutical Specialties. The employee may be working towards board certification as part of a timeline mutually agreed upon by the employee and supervisorLicensure as a Pharmacist in New York State required RESPONSIBILITIES Possess a broad scope and depth of pharmacotherapy knowledge and clinical skills. Independently responsible for complex decision-making, such as drug therapy selectionWorks in collaboration with others across the Health System providing patient care and medication consultation servicesAble to develop, recommend, and monitor therapeutic plansFunction on Health System multidisciplinary teams in the development of protocols, order sets, and critical pathways to optimize patient care. Works with pharmacy management and specialty departments to implement policies and procedures to ensure compliance with all applicable regulations Detect, prevent, monitor, document, and report adverse drug events and medication errorsRespond to patient care urgencies/emergencies within your area of expertiseProvide didactic and experiential training for Pharmacy Students, Pharmacy Residents, and other health care providers EDUCATION: BS: Pharmacy (Required)PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.LICENSES/CERTIFICATIONS: LPharm - Licensed Pharmacist - New York State Education Department (NYSED)PAY RANGE: $126,400.00 - $167,500.00 The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
