Adecco Permanent Recruitment is currently seeking a full time, direct hire Pre-Release Case Manager in Richmond, VA. This is for a full time role within a new but proven program, assisting incarcerated individuals prepare for release. Requirements:
Bachelor's degree in social service related field (social work, psychology, counseling, criminal justice, sociology, or similar)
1-3 years of experience implementing case management services and documenting interactions
Driver's license
Ability to pass DOC security clearance
CPR / Adult First Aid certification preferred
Job duties:
Facilitating excellent participant programming:
Conducting impactful 1:1 sessions, developing a trusting relationship with and comprehensive understanding of every participant on caseload
Upholding the quality and fidelity of Concordance Re-entry Model curriculum when delivering 1:1 sessions
Coordinating participant applications for Medicaid, food stamps, or SSI/SSDI
Preparing logistics to obtain necessary documents such as Social Security card, Birth Certificates, or state ID upon release
Reviewing the Participant Handbook to prepare for post-release programming
Offering Motivational Interviewing to keep participants engaged in the treatment process
Communicating with multi-disciplinary care team:
Documenting sessions in electronic record system, including noteworthy participant interactions and flagging participant needs
Working collaboratively with the Community and Life Skills team, pre-release career coach, and pre-release therapist assigned to each participant, providing input on progress to ensure consistency in instruction/learning pre-to-post release and alignment with participants' goals
Participating in internal meetings (Community and Life Skills team meetings, All Team meetings, 1:1 supervisor meetings, and training sessions)
Pay Details: $55,000.00 to $65,000.00 per year
