The Clinical Pharmacy Specialist - Ambulatory Care develops and coordinates pharmacy services in primary care, provides patient and population care management within a interdisciplinary team, contributes to quality improvement activities, promotes the safe, effective and cost-effective use of medications within the clinic. Patient care responsibilities: comprehensive medication management and medication review for high risk patients, interdisciplinary referrals for individualized medication and/or disease management and provider group-based medication management, both transitional care management and longitudinal care management services.

Population Health responsibilities: Analyze population health data to identify trends, gaps in care, and opportunities for intervention. Develop and implement population health initiatives targeting specific patient populations (e.g., those with chronic diseases, medication adherence, medication optimization). Monitor and report on health outcomes related to medication use.

Minimum Requirements:

Bachelor's of Science in Pharmacy from an accredited College of Pharmacy or a PharmD from accredited college of pharmacy (required)

Completion of an ASHP- accredited PGY1 Pharmacy Residency (required)

Completion of an ASHP- accredited PGY2 Pharmacy Residency (preferred)

Licensed Pharmacist in the state of PA and NJ (required)
