CareFirst NY LPN in Painted Post, New York
•Providing personal care, education and empowerment as outlined in the plan of care (POC).•Provides anticipatory guidance to patients/family throughout the hospice care and palliative care.•Accurately capture care provided according to the established standards; within our electronic medical records (EMR).•Provide emotional support to patients and family.• Work with and collaborate with all members of the Interdisciplinary team as needed.• Recognize and report situations of potential harm to Case Manager, Supervisor(s), TeamManager(s), and/or On-call nurse.• Participate in continuous care when needed.
Minimum Salary: 22.50 Maximum Salary: 27 Salary Unit: Hourly
