Join our mission as a Physical Therapist!We are looking for a physical therapist to treat patients for our neuro-vestibular population at the Onalaska Clinic! What you will work: 0.8 FTE = 32 hours a week Mondays - Fridays during clinic hours No weekends or holidays What you will do: Provide physical therapy in an outpatient clinic setting Work with patients and families to examine, evaluate, diagnose, prognose, and intervene to insure optimal outcomes Educates the patient and other caregivers as needed Collaborates with the referring provider and other clinical staff in providing optimal care of the patient through appropriate timely methods of communication What you get: An engaged team with a welcoming environment A top-rated retirement plan and healthcare benefits and much more A highly adaptable and mission-driven organization What you need: State of Wisconsin Physical Therapist License Current Infant-Adult BLS for Health Care Providers from American Heart Association or American Red Cross. Job Description: The Physical Therapist in their clinical role examines, evaluates and assesses individuals of all ages who have medical problems or other health related conditions that limit their ability to move or function with activities of daily living. The Physical Therapist develops a plan of care and provides treatment techniques and interventions to improve mobility, reduce pain, restore function and prevent disability. The Physical Therapist provides physical therapy services and elements of patient/client care management leading to optimal outcomes according to the relevant state Physical Therapy Practice Standards. The Physical Therapist works collaboratively with a multidisciplinary team in a team in the areas of education and consultation regarding patient needs in accordance with the vision and mission of Gundersen Health System. Major Responsibilities: 1. Works with patients and families to examine, evaluate, diagnose, prognose, and intervene to insure optimal outcomes. Also consults and educates the patient and other care-givers as needed. 2. The physical therapist works with the referring provider and other clinical staff in providing optimal care of the patient through appropriate timely methods of communication including, but not limited to, complete and accurate patient chart notes and face-to-face consultations. 3. Interacts and practices in collaboration with a variety of professions, communicates value of physical therapy services. 4.
