Salary: $40,000 - $42,000
Program Description:
Kenton Hall is a 108 bed 24/7 emergency shelter for homeless men with substance abuse issues and mental health issues. In addition to on-site rehabilitation services, our comprehensive services include case management, individual and group counseling, recreational activities and housing placement assistance. Additionally, an on-site medical, psychiatric and dental clinic operates Monday through Friday.
Overall Responsibilities:
Under the general direction of the Director of Social Services, the Case Manager is responsible for providing the required assessment, case management, and counseling services to a designated caseload of clients. The Case Manager works collaboratively with each client to assist them in identifying and securing community based housing as rapidly as possible.
Essential Duties and Responsibilities:
The essential duties of the Case Manager include but are not limited to the following activities:
Provides case management to a designated caseload of clients.
Conducts intake, psychosocial and housing assessments of clients.
In collaboration with each client, prepares and monitors an Independent Living Plan, identifying short and long term objectives to be achieved.
Inputs all client information and updates into the CARES system.
Counsels clients through individualized and group sessions on issues such as wellness self-management and independent living skills.
Identifies and makes referrals for community based treatment and support services.
With clients’ authorization, coordinates care with community based providers.
Works collaboratively with the Housing Specialist to identify and re-inforce each client’s housing plan.
Coaches/prepares clients for housing interviews.
Collaborates with on-site psychiatric provider, nursing, and primary care to ensure each client has an appropriate aftercare plan.
Qualifications:
High School/GED Diploma and experience as described below OR Associate's Degree
One to two years (1-2) year of experience working with homeless adults, substance use and/or mental health populations.
Teamwork, Organizational, Interpersonal, Communication, Computer, Bilingual a plus
All newly hired Project Renewal staff must provide verification of being fully vaccinated against Covid-19.
