OVERVIEW
The role of the Case Manager is to coordinate and conduct the Academy for Human Service Training (AHST) program for their assigned caseload by adhering to the proposal and contract requirements and accomplishing the goals set forth therein. The Case Manager assists in establishing and maintaining systems for monitoring and supporting the achievements of students and graduates in all aspects of the Academy for Human Service Training, including instruction, internships, certification, job development, job placement, and job retention. Statewide Travel required.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Works a flexible schedule that will include working both day and some evening hours at AHST program sites and/or in the community.
Assists students in determining their case management and programmatic needs and barriers to successful completion.
Serves as a liaison to area social service agencies. Assists in writing grant proposals to secure funding for the Academy.
Assists in the development of Individual Service Strategies for each student of the assigned Academy site to address areas of need and strength.
Counsels program participants regarding personal issues that may interfere with program completion and successful job attainment.
Prepares and submits various weekly & monthly reports to Director for assigned Academy sites.
Instructs specialized courses in areas of knowledge and expertise and covers course instruction in the absence of scheduled Instructors.
Conducts individual case management meetings with students and graduates of the Academy for Human Service Training.
Keeps accurate case notes on all individual and group meetings. Maintains student’s files according to the Academy Table of Contents and ensures documentation is accurate and objective.
Participates in outreach, information sessions, recruitment events, and interviews of potential students in an effort to increase referrals at all sites.
Ensures that consistent follow up is occurring with various customers, including current students, students who have graduated, collaborative providers, internship site supervisors, potential employers, employers where students may be placed upon completion, etc.
EDUCATION AND/OR EXPERIENCE :
Bachelor’s Degree in Human Services, Special Education, or related field plus two years’ experience with community-based employment, counseling, case management, and/or human resource development OR four years’ experience in the human services or related field with at least one year of that experience with community-based employment, counseling, case management, human resource development and/or DDS-funded programs OR equivalent combination of education and experience.
CERTIFICATES, LICENSES, REGISTRATIONS:
Valid Connecticut Driver’s License
