Actalent RN Case Manager in Moreno Valley, California

Job Title: RN Case ManagerJob Description

As an RN Case Manager, you will provide comprehensive case management for patients at skilled nursing facilities. This includes coordination with various team members like nursing, rehabilitation, dietary, and administration, along with discharge planning. You will also be responsible for telephonic case management in the Special Needs Department if required. This role involves conducting medical necessity screening, ensuring compliance with regulatory requirements, and coordinating care to promote efficient patient outcomes. You will work under the direction of a Registered Nurse, adhering to the scope defined by RN state licensure requirements.

Hard Skills

  • RN license

  • Discharge planning experience

  • Experience in an acute hospital setting (200-300 bed hospital)

  • HealthConnect or EPIC software experience

  • Excel data entry

Soft Skills

  • Efficient communication with interdisciplinary teams

  • Ability to manage concurrent disputes

  • Documentation skills

  • Resource utilization and management

Job Type

This is a contract position with a duration of 6 Month(s).

Work Site

This is a fully on-site position in Moreno Valley, California.

Work Environment

Monday to Friday with rotating weekends (every other weekend), typical shifts are 8am-5pm, with possible schedules including 11:00 AM to 07:30 PM or 08:00 AM to 04:30 PM.
