Department Overview The Infectious Disease and Immunology department provides services relating to the diagnosis and treatment of all acute, chronic, and recurrent infectious diseases. They also provide vacination inforamtion, professional consultation, clinical support for community providers, and various educational and outreach programs. Position Summary This position will collaborate with the Clinical Director and other OB and HIV psychosocial and medical staff to provide nursing care coordination for HIV-infected pregnant women, their HIV-exposed infants and their families in Minnesota. This position serves as an expert resource to patients, families, providers and the larger community. This position is grant-funded and dependent on continued funding. This position requires on-site and remote work. * Current MN RN license required. ACRN certification preferred. * Associate Degree in nursing required. Bachelor's preferred. * 2 or more years experience in an HIV care setting OR 2 or more years public health nursing experience OR 5 or more years relevant experience that includes essential job functions * Must have a working automobile. Mileage reimbursed based on federal rate. * Must possess a valid driver's license (must meet the minimum criteria of Children's Minnesota Fleet Safety Management policy) * Clean driving record * Must possess and provide verification of current automobile insurance coverage Preferred: * Previous care management of patients with chronic illness, preferably HIV * Experience working with HIV, Ryan White or other federally funded programs preferred * Experience working with diverse populations (racial, ethnic, socioeconomic status, LGBTQ) strongly preferred Knowledge/Skills/Abilities: * Demonstrated knowledge of perinatal HIV disease and related treatment, as well as primary care needs of HIV-infected patients * Skilled in client engagement, assessment, care development, service coordination, crisis intervention, complex medical discharge and placement planning and dealing with difficult issues sensitively * Demonstrated skills in effective written and verbal communication with staff, providers, patients/families and outside agencies * Strong critical thinking skills to work independently, establishing priorities, meeting deadlines and maintaining effective caseload management * Demonstrated ability to provide services in a non-judgmental, culturally sensitive manner * Strong computer skills * Conduct face to face client appointments through home visits and appointments at external care sites (requires use of personal car and includes driving throughout the state of Minnesota) Physical Demands Please click here to view the Physical Demands The posted pay range represents rates for a grouping of multiple jobs within a salary grade. We carefully consider a wide range of factors including but not limited to market indicators for the specific role, the skills, edu ation, training, credentials and experience of the candidate, and organizational needs. Base pay is just one piece of the total rewards program offered by Children's Minnesota.
