Mason Veteran Peer Outreach program; coordinate activities with multi-disciplinary team involved in managing outreach to patients and coordination of patient care including with physicians, psychologists, peer specialists, social workers and emergency/inpatient services. Primary Functions Monitor patient care from initial contact in the community through referral/coordination with outpatient, inpatient and/or emergency medical/psychiatric services to ensure patient progress towards desired outcomes. Collaborate daily with peer specialists in the community to maximize quality care. Assist peer specialists with patient assessments. Communicate plan of care to patients including responding to emails, media posts, telephone messages and facilitating referral to peer specialists, outpatient care and/or emergency/inpatient services. Coordinate patient ongoing contact to support follow-up and referral to clinical services. Complete program evaluation summaries. Facilitate staff meetings and advisory committee meetings. Assist with the fiscal management of annual budgets. Provide outreach/education services to other professionals as appropriate. Knowledge - Skills - Abilities Knowledge of biology, clinical judgment, medicine, cultural awareness, and consulting. Medical documentation, interpersonal, practice-based learning and improvement, empathy, and decision making skills.
