The Department of Medicine, Division of Gerontology and Geriatric Medicine has an exciting opportunity for a Full-Time Social Worker to work on the UHealth Campus. The Social Worker will provide psychosocial assessments, intervention, and/or treatment and discharge planning to patients and families to assist them in coping with their hospitalization, illness, diagnosis, and/or life situation and meet the continuing needs of patients post hospitalization. The Social Worker administers a broad variety of therapies and are the designated experts, consultants, or sources of knowledge within the social work profession.

  • Provide psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and/or life situation.

  • Formulate, develop, and implement a plan utilizing appropriate social work interventions.

  • Collaborate with the health care team and involve the patient/family in the development and implementation of plans. Facilitate discharge planning across a multidisciplinary care continuum which includes interacting with consulting physicians, third party payer and other health care providers to facilitate movement of the patient through the system and arrangement of an appropriate discharge plan.

  • Works collaboratively with the multidisciplinary staff to design and implement treatment plans and discharge plans for the patient.

  • Provide education and counseling to patients and families around issues related to adaptation to the patient's illness and/or life situation. May utilize crisis intervention, brief and long-term individual, group, and family therapies.

  • Administers a broad variety of therapies. Are the designated experts, consultants, or sources of knowledge within the social work profession.

  • Provide education to other health care professionals.

  • Participates in or consults on research/education programs.

  • Supervises MSW students.

  • May function as a resource to other staff as appropriate.

MINIMUM QUALIFICATIONS

  • Master’s Degree in Social Work

  • Minimum 1 year of experience

  • Florida license in Social Work required.

  • Excellent written and verbal communication skills.

  • Strong interpersonal skills; ability to collaborate well.

  • Solid knowledge of social work practice: strong assessment, interview and counseling skills.

  • Ability to handle difficult and stressful situations with professional composure.

  • Ability to work independently and/or in a collaborative environment.
