Schoolcraft Memorial Hospital Patient Navigator in Manistique, Michigan
POSITION SUMMARY: The Patient Navigator (PN) acts as the liaison between the patient’s health care providers and the community. This person specializes in social determinants of health (SDOH) and plays a critical role in addressing the non-medical factors that influence health outcomes. This position is essential for connecting patients to community resources that can help improve their overall well-being by addressing issues such as housing, food insecurity, transportation and social support.
QUALIFICATIONS AND EXPERIENCE:
Experience in a customer service setting, preferably in a medical office or health care facility.
Minimum of an associate’s degree in health care, LPN, MA or counselor.
Complete Provider Delivered Care Management (PDCM) training within one-year employment.
Experience using Microsoft Office Programs and Electronic Medical Record system(s) preferred.
BLS Certified
Clear and concise verbal and written communication; maintain professional telephone etiquette
Ability to respond to stressful/emergency situations or frequent interruptions in a calm and effective manner, ability to multi-task
Maintains confidentiality and protects sensitive data at all times.
Adheres to organizational and department specific safety standards and guidelines.
Works collaboratively and supports efforts of team members.
Demonstrates exceptional customer service and interacts effectively with physicians, patients, residents, visitors, staff and the broader health care community.
Knowledge of medical terminology.
DUTIES AND RESPONSIBILITIES:
The PN will be the point of contact between the community needs and the clinic resources acting as a resource specialist.
Provide care coordination to patients, including assistance with payor regulations for this service.
Oversees SDOH (Social Determinants of Health); engages patients in care coordination to meet social needs including housing, food, utilities and transportation assistance.
Network in the community and develop and distribute appropriate resource materials
Facilitate patient access to appropriate medical care including substance use disorder, mental health, dental health and/or medical providers. Educates patients on importance of keeping appointments and assists with this process.
Participates in department and interdepartmental process improvements; recommends improvements in clinical process as opportunities are identified. Attends daily huddle and monthly staff meeting for the clinic.
Maintain tracking systems for identified patients and quality of care performance measures.
Attend conferences/workshops to enhance/maintain expertise.
Demonstrates appropriateness in meeting objectives in age-specifics.
Perform other duties as may be assigned.
