Schoolcraft Memorial Hospital Patient Navigator in Manistique, Michigan

POSITION SUMMARY: The Patient Navigator (PN) acts as the liaison between the patient’s health care providers and the community. This person specializes in social determinants of health (SDOH) and plays a critical role in addressing the non-medical factors that influence health outcomes. This position is essential for connecting patients to community resources that can help improve their overall well-being by addressing issues such as housing, food insecurity, transportation and social support.

QUALIFICATIONS AND EXPERIENCE:

  • Experience in a customer service setting, preferably in a medical office or health care facility.

  • Minimum of an associate’s degree in health care, LPN, MA or counselor.

  • Complete Provider Delivered Care Management (PDCM) training within one-year employment.

  • Experience using Microsoft Office Programs and Electronic Medical Record system(s) preferred.

  • BLS Certified

  • Clear and concise verbal and written communication; maintain professional telephone etiquette

  • Ability to respond to stressful/emergency situations or frequent interruptions in a calm and effective manner, ability to multi-task

  • Maintains confidentiality and protects sensitive data at all times.

  • Adheres to organizational and department specific safety standards and guidelines.

  • Works collaboratively and supports efforts of team members.

  • Demonstrates exceptional customer service and interacts effectively with physicians, patients, residents, visitors, staff and the broader health care community.

  • Knowledge of medical terminology.

    DUTIES AND RESPONSIBILITIES:

  • The PN will be the point of contact between the community needs and the clinic resources acting as a resource specialist.

  • Provide care coordination to patients, including assistance with payor regulations for this service.

  • Oversees SDOH (Social Determinants of Health); engages patients in care coordination to meet social needs including housing, food, utilities and transportation assistance.

  • Network in the community and develop and distribute appropriate resource materials

  • Facilitate patient access to appropriate medical care including substance use disorder, mental health, dental health and/or medical providers. Educates patients on importance of keeping appointments and assists with this process.

  • Participates in department and interdepartmental process improvements; recommends improvements in clinical process as opportunities are identified. Attends daily huddle and monthly staff meeting for the clinic.

  • Maintain tracking systems for identified patients and quality of care performance measures.

  • Attend conferences/workshops to enhance/maintain expertise.

  • Demonstrates appropriateness in meeting objectives in age-specifics.

  • Perform other duties as may be assigned.
