Schoolcraft Memorial Hospital LPN - Homecare and Hospice in Manistique, Michigan
POSITION SUMMARY: Provide safe, individualized nursing care/procedures According to the nursing care plan and in adherence with Schoolcraft Memorial HomeCare policies and procedures and all Federal, State and local laws/regulations.
QUALIFICATIONS:
Graduate of an accredited school of nursing.
Currently licensed in the State of Michigan.
One year of recent HomeCare experience preferred.
Good mental and physical health.
Satisfactory references.
Passing score on medical administration exam.
Able to provide own transportation to patient’s homes.
Able to work beyond scheduled hours as needed.
Current CPR Certificate.
WORK CONDITIONS:
Requires driving/travel to other sites, including in inclement weather. Company vehicle may be provided.
Lifting and transporting of patients, supplies and equipment.
Occasional exposure to infectious and contagious entities.
Occasional exposure to unsafe/unsanitary conditions in patient homes.
Notebook computer used for HomeCare client documentation.
DUTIES & RESPONSIBILITIES:
The services provided by a Licensed Practical Nurse for home health, hospice clients include, but are not limited to:
Provide safe, individualized nursing care/procedures according to the nursing care plan and in adherence with Schoolcraft Memorial HomeCare policies and procedures and all Federal, State and local laws/regulations.
Documents assessments, observations, and care/procedures provided to the patient completely, accurately, legibly. Notifies Case Manager/Clinical Supervisor immediately of any changes in patient’s status.
Prepares, administers, and documents medications according to Schoolcraft Memorial HomeCare policies and procedures.
Accompanies patient to physician appointments. prn
Maintains patient’s immediate environment to be safe and clean.
Assists the patient’s family and other health care team members in providing continuity of care and helping to meet the emotional needs of the patient and immediate family.
Demonstrates the ability to organize patient care activities.
Willing and able to meet consistently the patient visit frequency expectations in accordance with Schoolcraft Memorial HomeCare’s productivity guidelines.
Organizing work to adequately perform patient care activities.
Contributes to professional standards of care at Schoolcraft Memorial HomeCare by:
Collaborating with management to identify issues that require further exploration.
Participating in and supporting the Quality Assurance Program and Utilization Review activities.
Participating in Schoolcraft Memorial HomeCare meetings, activities, programs, inservices, and continuing education classes.
Remaining flexible to changes in Schoolcraft Memorial HomeCare policy and procedure and in daily scheduling.
Maintaining collegial relationship with other disciplines, interpreting the role and responsibilities of the Schoolcraft Memorial HomeCare LPN to others.
Maintaining regular work attendance.
Enforces Schoolcraft Memorial HomeCare policy and procedure by:
Communicating and interpreting the policies and procedures to members of the Schoolcraft Memorial HomeCare team, patients, and families.
Communicating with management if policies and/or procedures are violated.
Follows agency policy and procedure for emergency.
Contributes to effective consumer relations by assisting patients, families, co-workers, and other health care members to resolve expressed concerns and by demonstrating a positive and helpful attitude.
Minimizes infectious diseases among employees and patients by following infection control policies and protocols. Attends on-going inservice education programs regarding safety issues in the home, equipment, and universal precautions.
Keeps information confidential by adhering to the terms of Schoolcraft Memorial HomeCare Human Resource Policy concerning confidential information.
Demonstrate appropriateness in meeting objectives in age-specifics.
Performs other related duties as may be required.
