JOB REQUIREMENTS: Position: Caregiver Reports to: Executive Director Shifts Available: Monday/Wednesday/Friday 7:00 AM to 7:00 PM Monday/Wednesday/Friday 7:00 PM to 7:00 AM Tuesday/Thursday/Saturday 7:00 AM to 7:00 PM Tuesday/Thursday/Saturday 7:00 PM to 7:00 AM Wage: \$20-26/hour. Wage determined by level of relevant experience, education, certification, licensure, skills, and training. Wage differential for overnight shift and holidays. Hiring Bonus: \$250 hiring bonus available upon hiring for eligible Certified Direct Care Professionals (CDCP). Additional \$250 bonus provided after 6 months of employment for eligible CDCPs. Caregiver- Job Description Organizational Background and Position Summary Solace Home is the keystone initiative of Solace Friends, Inc., a non-profit organization working to bridge the existing gaps in end-of-life care for individuals experiencing homelessness or housing insecurity. Solace Home is a licensed 4-bed Adult Family Home in Madison that provides housing and caregiving support for persons with a terminal illness who are experiencing homelessness, housing insecurity, or extreme poverty. Solace Friends/Solace Home is not a hospice provider. All Solace Home residents will receive hospice care from community hospice agencies and Solace Home caregivers will serve as the residents \"family\", providing assistance with activities such as bathing, toileting, dressing, transfers, and mobility, as well as companionship, meal preparation, cleaning, and laundry. Solace Home Caregivers will embody empathy and compassion to provide residents with person-centered care that nurtures their comfort and dignity. Caregiver Responsibilities Resident Care Work closely with community hospice care providers to implement the personal hospice care plan for each Solace resident Provide assistance with activities of daily living (ADLs) such as bathing, toileting, getting dressed, ambulation, eating/feeding, personal hygiene Perform simple health tasks such as wound care, ostomy care, catheter care, and nail care as directed by the licensed hospice personnel Assist residents with administration of medications as directed by the licensed hospice personnel Report any concerns or changes in residents\' health to supervisor and hospice care provider in a timely manner in order to make modifications to the care plan Provide person-centered end-of-life and spiritual and emotional support to residents Administrative Complete thorough and accurate documentation of medication administration, resident care activities, and other duties Assist in Solace Home administrative duties as trained and requested Household Support Assist with household duties including, but not limited to meal preparation, cleaning, laundry, and shopping Perform other Solace Home household duties as trained and requested Position Qualifications Demonstrated proficiency (through work experience, education or training/certification) in one or more of these areas is preferred: Experience working with low-income or homeless populations Experience working with terminally ill individuals Experience working with individuals with mental health and/or substance abuse histories Experience working in an adult family home or community based residential facility Required Knowledge and Skills Ability to lift 50lbs High school diploma Prior work experience/certification/training as Certified Direct Care Professional (CDCP) or Certified Nursing Assistant (CNA) preferred but not required. Medication Aide certification preferred but not required. CNA, CDCP, and Medication Aide training will be provided and paid for by Solace Home for those without prior experience as determined by the needs of Solace Home upon successful hiring.
