UCLA Health RN Care Manager - Ambulatory Care Management, Hybrid-Remote in Los Angeles, California

Description

As an RN Care Manager, you will function as a clinician, advocate, and educator to achieve optimal clinical and quality outcomes by effectively managing care and resources to reduce unnecessary utilization. The RN Care Manager utilizes clinical expertise and understanding of care management, CMS and NCQA regulatory requirements, quality measures of care, and provides safe and appropriate patient transitions across different levels of care. The RN Care Manager develops person-centered care goals, identifies barriers to health and implements interventions to meet patient goals through development of an individualized care plan with emphasis on promoting patient self -management and family responsibility.The RN Care Manager identifies and ranks relative importance and urgency of potential problems and plans contingencies. Accepts ownership of problems and willingly engages in the problem solving process, as an individual and as a group member. They maintain a positive attitude, when dealing with adversity and challenging situations and adapts to change effectively.

Salary range: $125,259 - $161,987/Annual

Qualifications

Required:

  • Current unrestricted California RN license.

  • BSN Degree

  • 5 years of Case Management experience

  • Must have strong problem solving skills; ability to analyze problems and formulate plans, solutions and a course of action given changing circumstances.

  • Excellent organizational and time management skills, including the ability to handle multiple conflicting priorities simultaneously. Strong ability to prioritize.

  • Demonstrate proficiency in computer skills such as Windows computer function, computer navigation, and ability to use computerized patient care systems.

  • Competence in navigating the internet/intranet and use of browser software such as Microsoft Explorer and applications such as Excel, Word, and PowerPoint, MS Teams, Care-Connect/ EPIC, Zoom, ICAP, Right-Fax, AIDIN, and JABRA Headset.

  • Excellent interpersonal, verbal and written communication skills. Ability to articulate clinical and non-clinical information to individuals of all levels of understanding.

  • Ability to be flexible and work according to availability of office personnel, patient and resources required to coordinate care of patient.

Preferred:

  • MSN degree

  • CCM or ACM
