The USC Alfred E. Mann School of Pharmacy is seeking a highly skilled and motivated Clinical Staff Pharmacist to join our team. This unique position will be divided between two prestigious institutions: The USC Mann School of Pharmacy’s California Right Meds Collaborative (CRMC) and Cedars Sinai Medical Center. Additionally, the Clinical Staff Pharmacist will play a vital role in a CDC grant from the Los Angeles Department of Public Health focusing on heart disease prevention and management. The percent effort is 50% each for CRMC and Cedars-Sinai.

Responsibilities:

USC California Right Meds Collaborative (CRMC)- 50%:

  • Collaborate with various healthcare stakeholders (health plans, hospitals / health systems, clinics, pharmacies, etc.) to optimize medication therapy for patients enrolled in CRMC

  • Serve as a mentor for CRMC team members to ensure optimal delivery of comprehensive medication management services

  • Lead efforts to improve patient engagement, activation, and retention in CRMC

  • Participate in interdisciplinary team meetings to discuss patient cases and develop personalized care plans

  • Assist CRMC teams with complete, accurate, and timely documentation necessary for continuous quality improvement and impact evaluation / program effectiveness

  • Serve as lead for CDC grant deliverables in partnership with the Los Angeles County Department of Public Health.

  • Developing & managing provider partnerships that are high-value to health plans. This includes but is not limited to fostering trusted relationships with providers/medical leadership/medical groups, attending meetings with physicians and staff to promote CRMC services and provide updates/status/reporting, establishing Collaborative Practice Agreements (CPAs) with partnered providers, supporting other pharmacy related needs/gaps for providers (e.g. providing in-services, etc.), facilitating access to electronic health records, and providing in-person CMM at the medical provider location on a weekly basis if feasible.

  • Actively participating in and contributing to all CRMC related meetings (e.g. management and planning meetings, 1:1s, training sessions, learning sessions, continuous quality improvement meetings, etc.)

  • Promptly respond to inquires from health plans, CRMC staff and faculty, medical partners, and other key CRMC stakeholders

  • Provide training and guidance to pharmacy staff (pharmacists, pharmacy technicians, intern pharmacists / students, etc.) on delivering community health worker services

Cedars Sinai Medical Center (CSMC)- 50%:

  • Provide comprehensive clinical pharmacy services to neurology service patients, in collaboration with the neurology team, including comprehensive medication management. Examples include optimization of medication therapy, identification and resolution of drug-related problems, and prevention and management of adverse drug events.

  • Serve as an expert resource for medication-related information for patients and healthcare professionals.

  • Participate in relevant committees and quality improvement initiatives to enhance medication safety and patient outcomes.

  • Collect, analyze, and report data demonstrating quality improvement and impact of services provided.

  • Participate in or lead scholarly activities with the neurology team including grantsmanship, publications, and presentations at professional meetings.

The candidate will also participate in precepting, leading, and supervising APPE/IPPE students/intern pharmacists, pharmacy technicians, and/or pharmacy residents.

This position will have a direct report to the Associate Dean for Clinical Affairs at USC Mann, and a dotted line report to the Vice President and Chief Pharmacy Officer at CSMC.

Required Qualifications:

  • Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy.

  • Active pharmacist license in the state of California.

  • Strong clinical knowledge and expertise in medication therapy management.

  • Excellent communication and interpersonal skills.

  • Ability to work effectively in interdisciplinary teams and adapt to changing environments.

  • Commitment to patient-centered care and continuous professional development.

  • Proficient in basic Microsoft Office applications including Word, Excel, PowerPoint

Preferred Qualifications:

  • Completion of a pharmacy residency program

  • Previous experience in providing and documenting medication management services

  • Previous experience in precepting and teaching APPE/IPPE students/intern pharmacists, pharmacy technicians and pharmacy residents

  • Multilingual, particularly Spanish

Minimum Education: Bachelor's degree Minimum Experience: 1 year Minimum Field of Expertise: Degree from accredited (ACPE) school of pharmacy. Registered pharmacist in state of California. Hospital pharmacy experience. Familiarity with unit dose, IV admixtures and clinical pharmacy practice.
