Required: * High school diploma or GED * 3-5 years of related experience in a healthcare environment * Computer skills, including database, spreadsheet, word processing, communicate by e-mail and scheduling software Summary: This position is responsible for assisting in the optimization of the Behavioral Health (BH) department through supporting Behavioral Health Transition of Care (TOC) needs, providing care coordination support of the Behavioral Health Case Managers, and completing front-end behavioral health utilization management processes. This position is also responsible for providing supporting in the completion of special projects. Essential Duties: 1. Provides Transition of Care (TOC) support to BH Case Management staff and the Director of the department through daily care coordination activities, including surveillance of local arrest reports and hospital event notifications, and documentation applicable information in the care of management system 2. Completes special projects or deliverables as assigned 3. Indexes and creates BH authorizations for review in the appropriate software program, including working with providers to obtain appropriate and complete documentation 4. Manages and maintains BH authorization and BH case management communications within the case management platform Duties Include: * Works closely with BH Case Managers to ensure accurate, complete, and timely documentation has been received for the completion of case management and utilization review tasks * Represents the organization in assigned community meetings and collaborative care and Interdisciplinary Team (IDT) meetings * Performs administrative tasks which include filing, scanning, database management, and data entry * Prepares written correspondence for member communications as needed * Executes all duties in accordance with State and Federal requirements to achieve optimal outcomes for members * Assist with preparation of tasks and reports related to quality metrics, audits and deliverables to the OHA * Prepares member correspondence and other written communication as needed * Triages telephone calls and routes to appropriate staff as necessary * Proactively identifies and resolves issues to promote positive outcomes for members * Performs member outreach duties as assigned * Maintains confidentiality and complies with HIPAA rules and regulations * Maintains punctual, regular and predictable attendance * Works collaboratively in a team environment with a spirit of cooperation * Displays excellent communication skills including presentation, persuasion, and negotiation skills required in working with members, providers, and community partners and including the skill to communicate effectively and remain calm and courteous under pressure
