JOB TITLE: Registered Nurse or Licensed Practical Nurse (LPN)

JOB SUMMARY: The Licensed Practical Nurse shall have the personality, ability, and temperament necessary to interact with supported persons with diverse-abilities in a manner that respects and maintains the individuals' spirit, dignity, and individuality. This role is responsible for providing care and support within the scope of their nursing practice and assisting in the daily operation and leadership of the community inclusion service to promote independence, learning, and self-expression according to the abilities and choices of supported persons.

REPORTS TO: Director of Service and Regional Manager

KEY DUTIES & RESPONSIBILITIES:

  • SERVICE TO SUPPORTED PERSONS

    • Ensure the implementation, regular review, and updating of individualized program plans to address the total needs of each supported person.
  • Communicate with fellow RNs/LPNs regarding assessments, required monitoring, necessary nursing care, and other relevant information.

  • Update medication administration records (MAR) and medication change sheets as needed.

  • Plan and evaluate nursing care and therapies for supported persons, collaborating with physicians, professional therapists, social workers, and other experts.

  • Collaborate with team members without nursing backgrounds regarding medications, medication side effects, supported persons' diagnoses, and nursing care procedures, ensuring they can identify and report matters that require monitoring.

  • Continuously assess and manage care as needed.

  • Advocate for the active participation of each person living in the community.

  • Provide necessary assistance and instruction to supported persons in all aspects of their daily lives.

  • Ensure that individuals have opportunities for realistic choices in community recreational and leisure activities, tailoring these activities to their preferences.

  • Support individuals in developing and maintaining family and personal relationships.

  • Serve as a liaison between individuals' homes, families, and community resources.

  • Monitor the health and safety of supported persons and implement preventive measures.

  • Maintain a clean and tidy living environment at all times.

  • ORGANIZATION

    • To provide quality service for the supported persons including maintaining standards of care and the health and safety of the persons.
  • To attend any planning meetings including the preparation for and follow-up of each meeting as required.

  • To give as much notice as possible when unable to attend work shifts and meetings, ideally, a minimum of 24 hours is required.

  • To be responsible for completing all personnel forms including time sheets, expense claim sheets and mileage sheets, etc. for Coordinators' /Managers' approval.

  • To be available for additional shifts as required.

    QUALIFICATIONS AND SKILLS:

    • Current registration with the BCCNM or EMALB as an RN or LPN
  • Previous experience in a healthcare setting or recent graduation from a nursing program with a multidisciplinary approach.

  • Experience in Individualized Program Plans (IPP), Person-Specific Planning (PSP), and Health Care Planning (HCP).

  • Current CPR "C" certification.

  • Valid Class 5 driver's license; Class 4 driver's license preferred.

  • Strong team collaboration skills.

  • Effective communication with staff, professionals, and families.

  • Ability to make clinical judgments based on current nursing standards.

  • Conscientious therapeutic practice.

  • Ability to self-evaluate performance critically.

  • Any qualifications or skills required by the Licensing officer.
