Position Details

POSITION OVERVIEW

TheHospice RN Clinical Directoris a qualified professional responsible for the planning, coordination, and delivery of quality care services to the client. This individual is accountable for the agency’s adherence to federal, state, and local laws, accreditation, company standards, compliance, and reimbursable client care services.

Essential Job Fuctions:

  • Communicates with the primary physician to evaluate total patient/family needs and establishes the Hospice Plan of Care within 24 hours of admission.

  • Oversees assignment of each patient/family to appropriate field staff within 24 hours of admission.

  • Coordinates the delivery of patient/family care with the interdisciplinary team members including the Medical Director and attending physician.

  • Provides for regular interdisciplinary team meetings for the development, review and revision of the patient/family plan of care, including exchange of information, review of problems, assessing effectiveness of interventions and documentation of outcomes.

  • Ensures the timely and adequate delivery of Hospice services to patients and families through ongoing evaluation of interventions, goals and outcomes.

  • Ensures new staff orientation to job duties by assisting in the orientation plan, providing ongoing supervision and evaluation, establishing preceptors as needed.

  • Identifies need for staff education and assists in the development of in-service programs.

  • Supervises direct care delivery activities of team members for conformity to establish standards of care as set forth by the Standards of Hospice Nursing Practice, Medicare regulations, Joint Commission standards and other professional licensing bodies.

  • Reviews patient care documentation, oversees ongoing audit of patient records, resolution of deficiencies, and routinely reviews audit findings.

  • Oversees the scheduling of 24 hour nursing coverage for hospice patients.

  • Promotes quality oriented and cost effective utilization of hospice resources.

  • Makes visits to patients/families as indicated by patient/family need, staffing resources, and supervisory requirements.

  • Participates in recruitment and selection of professional and non-professional field staff, performs probationary and annual performance reviews of supervised staff.

  • Participates in on call rotation for delivery of care, after office hours, on weekends and holidays.

  • Maintains sound privacy and security practices and prevents privacy or security breaches. If breach occurs, takes corrective action.

  • Other duties as assigned by supervisor.

Position Requirements:

  • Current license as a Registered Nurse in the State of Application.

  • Valid CPR certification

  • Valid Drivers License

  • Minimum of two years of supervisory experience in a hospice setting.

  • Demonstrated ability to manage all aspects of terminal care, including family counseling and support, pain and symptom management, and bedside nursing.

  • Has a thorough knowledge of Medicare regulations, Joint Commission standards governing the delivery of Hospice services.
