Local Home Health Care Provider is hiring a Physical Therapist for in home visits.
Pay varies dependent on visit:
Routine Visit $55.00
Recertification $100.00
Start of Care: $165.00
Job Description: Under the general supervision of the Clinical Director, the Home Health Physical Therapist delivers care to clients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation. The PT effectively interacts and provides information on health maintenance and disease prevention, and provides case management with clients, significant others, and other health team members while maintaining standards of professional care.
Minimum Qualifications: Graduate of an accredited school of Physical Therapy. Current, unencumbered state license to practice as a Physical Therapist (PT) Minimum one (1) year of PT or PTA experience required with home health experience preferred. Reliable transportation
Essential Functions: The physical therapist shall be responsible for the following:
1) The Physical Therapy Evaluation
2) Regularly reevaluating the consumer's needs
3) Initiating and/or reviewing the plan of care and necessary revisions
4) Furnishing those services requiring substantial and specialized skill
5) Initiating appropriate preventive and rehabilitative procedures
6) Preparing clinical notes, coordinating services, and informing the physician and other personnel of changes in the consumer's condition and needs
7) Counseling the consumer and family in meeting physical therapy and related needs
8) Participating in in-service programs, supervising and teaching Physical Therapy Assistants (PTA) and other personnel.
Working Conditions: Client home setting, automobile travel and office setting. Contact with blood or other body fluids may pose a risk for exposure to blood borne pathogens and infectious diseases.
Physical Demands: Ability to sit, stand, bend and stoop for periods of time. Ability to exert up to 25 pounds of force occasionally. Ability to work in a stressful environment. Ability to respond to emergency/crisis situations. Exposure to blood and/or body fluids. May include extended time at a computer work screen and on the telephone. Other duties may be assigned.
