CENTRAL UTAH COUNSELING CENTER Case Manager in EPHRAIM, Utah

TITLE: CASE MANAGER

DEPARTMENT: Central Utah Counseling Center

JOB DESCRIPTION:

  1. Assist with transportation of consumers when required.

  2. Assist consumers in acquiring various services they need and require.

  3. Develop a working knowledge of support services available to the consumer and resources available such as SSI, SSA, Medicaid, housing, employment, etc.

  4. Develop and write a service plan for each consumer.

  5. Develop a supportive relationship with consumers.

  6. Maintain contact with consumers ranging from daily visits to once a month contact depending upon consumer needs.

  7. Be able to work appropriately with an interdisciplinary team.

  8. Assist family of consumers as required and appropriate.

  9. Advocate for rights of consumers.

  10. Work in coordination with other mental health/substance abuse service providers to assure consumer needs are being met.

  11. Maintain accurate records and reports.

  12. Maintain strict confidentiality with information pertaining to clients.

MINIMUM QUALIFICATIONS:

  1. Must have a minimum of a high school degree.

  2. Must be able to perform moderate physical activity.

  3. Must be able to physically assist handicapped individuals.

  4. Must be able to work flexible hours to occasionally include evenings and weekends.

  5. Must have a valid drivers license.

  6. Must pass the case management test with a score of 90% or better.
