Description Lutheran Social Services of WI and Upper MI is seeking a Forensic Care Coordinator to serve Walworth and Jefferson Counties Our Care Coordinators provide case management for individuals that have been convicted of crimes, are in prison and have Diagnostic and statistical manual mental health diagnosis . This role collaborates closely with the Department of Corrections to help individuals released from the correctional institution reenter the community successfully. The individual will be required to meet clients in the prison system and in the community to perform assessments, investigations/screenings to determine entry and placement in the Forensic Opening Avenues for Reentry Success Program. This is a full time, benefit eligible remote role! Some remote capabilities. No weekends! Under direction, performs work involving the assessment, service planning, and implementation of the service plan for assigned individuals. Performs other duties as required.
