Position Description:
The case manager serves as counselor and resource person for a caseload of patients. Specific duties include: providing individual and group counseling; participating as a member of a treatment team; corresponding with outside agencies, institutions and families of patients; collecting and writing social histories; monitoring patient/client treatment plan reviews; and compiling staffing notes. Functions under the immediate supervision of a supervising psychologist, social worker, or physician.

Major/Essential Functions:
Assumes the role of counseling therapist for individuals, couples and families with psychiatric and medically referred issues and health related problems, such as chronic illness, psychosomatic illness, adjustment disorders
Coordinates care with medical providers, coordinate community service resources with patient needs
Consults with medical providers regarding mental health interventions with patients
Perform urgent mental health assessment and consult
Management of patients with personality disorders
Develop, maintain and lead the behavioral health curriculum
Coordinate, maintain and lead behavioral health curriculum
Coordinate the psycho-social topics in the conference schedule
Teach or facilitate teaching of behavioral and mental health topics relevant to board reviews
Performs other tasks and duties assigned by supervisor within the policies of the department and TTUHSC in order to meet the goals and objectives of the program and the department
Adhere to all TTUHSC-EP policies, procedures, and processes
Personally demonstrate, display, and act in accordance with TTUHSC-EP's values (Service, Respect, Accountability, Integrity, Advancement, and Teamwork)
Actively promote and encourage staff across the institution
Perform all other duties as assigned

Preferred Qualifications:
Masters in Social Work
Minimum of 2 years of family therapy or counseling experience with a diverse clientele covering the age spectrum (pediatrics to geriatrics)
Previous experience with educational responsibilities for health profession learners including writing behavioral health curriculum, working with preceptors, and presenting at educational conferences is highly preferred
Excellent writing and verbal skills
Has the ability to multitask and prioritize and work in a multi-disciplinary team
Previous clinical or medical social work experience in a medical setting is highly preferred
LCSW or current eligibility and completed licensure in the state of Texas within 6 months
Bilingual in English and Spanish
