Geisinger RN - Registered Nurse - Infusion Pharmacy Coordinator in Danville, Pennsylvania

Job Summary

Collaborates with the multidisciplinary team to provide quality, efficient, accurate and safe home infusion therapy services in compliance with accrediting bodies. Assesses, plans, implements, coordinates, monitors and evaluates the effectiveness of the infusion plan of care and ongoing educational and associated support needs.

Job Duties

  • Call Patients weekly to set up next delivery of medication

  • Provides education with each phone call

  • Write and send a progress note to the provider per the policy

  • Completes telephonic teaching to patients regarding the medication they will be receiving

  • Must know all medications that the pharmacy provides so questions from patients can be answered

  • Answer phones/triage calls. provide education to the caller or transfer if needed

  • Communicate with the pharmacist and warehouse.

  • Check CADD pumps for infusion and deliver to the warehouse.

  • Create a patient folder for each new patient coming on the service

  • Works with leadership team as directed to review and develop infusion protocols, standards of care, procedures and patient education material.

  • Assesses the clinical, infusion related, educational and psychosocial needs of the client and caregiver.

  • Designs individualized plans of care with the client and caregiver.

  • Promotes an environment of collaboration and team when dealing with the client and caregiver, managing provider, home health agency and other members of the health care team to ensure safe and effective home infusion therapy services.

  • Prepares appropriate educational materials for each client and implements clinical and educational interventions based on identified needs and evidence-based guidelines.

  • Accurately and as per policy documents in the medical record to adequately reflect assessments, plans of care, interventions, collaboration and evaluation of therapy compliance and effectiveness.

  • Collaborates with the physician and other health care professionals to develop home infusion protocols to meet each client's specific infusion therapy needs.

  • Is able to respond to the concerns and complaints of clients and their caregivers, physicians, referral sources or home health agencies by utilizing appropriate listening techniques then referring the individual and group to the appropriate resource for resolution of the issue.

  • Performs case management and consulting functions with all members of the health care team.

  • Cooperates with and shares responsibility with other nursing staff for covering week day, holiday, and weekend on call services to ensure 24 hour a day and 7 day a week coverage.

Position Details

  • Hours: 8:30 AM to 5 PM

  • Weekend rotation: about every 12th weekend based on staffing

  • On-call: about 5 nights per 12 weeks based on staffing

  • Holiday rotation: 1 holiday per year based on staffing

  • Required experience: 3 years of RN work experience

  • Preferred experience: home health or case management

Education

Graduate from Specialty Training Program-Nursing (Required), Bachelor's Degree-Nursing (Preferred)

Experience

Minimum of 3 years-Nursing (Required)

Certification(s) and License(s)

Licensed Registered Nurse (Pennsylvania) - RN_State of Pennsylvania; Basic Life Support Certification - Default Issuing Body
