Case Manager (SOR)

The SOR (State Opioid Response) Care Manager will be responsible for providing services with individuals pre-admission. The care manager may work with admitted clients who are struggling to maintain engagement in treatment and are at higher risk. The SOR Care Manager will provide timely outreach and engagement for individuals referred from entities such as emergency departments and hospitals, law enforcement, county jails, etc., This position is responsible for managing an assigned caseload of clients and assessing needs, developing, implementing, and reviewing care management plans, and working to meeting/achieving client service needs. This care manager will be expected to work collaboratively with peer recovery specialists in conducting pre-admission services.

Develop and implement a Recovery Plan to help initiate and sustain an individual/family in recovery from substance use disorder.

Maintains caseload and submit timely, detailed records.

Utilize various techniques to engage clients and minimize failed appointments.

Assist clients with applying to referral agencies/programs and for benefits and with travel training, accompany clients to court appearances and medical or other appointments.

Assist clients with identifying and overcoming any barriers.

Provide non-clinical crisis support and education.

Transportation for participants to/from treatment or recovery support activities

Effectively utilizes agencys medical records software program(s) to complete required documentation.

Employee must be able to prioritize situations and manage time and may find that they are needed at irregular hours. They may have to manage physically threatening clients and be prepared to be responsible for their own safety, and the safety of others.

Work Environment

The employee is expected to adhere to strict policies/regulations relative to being in a tobacco and alcohol-free workplace. Work is performed both in an office environment and out in the community.

Qualifications

Must meet one of the educational criteria and one of the experience criteria listed below:

  1. Education

a. A Bachelors degree with a major or concentration in one of the following fields: Social work; Psychology; Nursing; Rehabilitation; Education; Occupational therapy/Physical therapy; Recreation or recreation therapy; Counseling; Community mental illness; Child and family studies; Sociology; Speech and hearing; Other human services field;

b. A NYS teachers certificate with bachelors degree or above

c. NYS licensure and registration as a Registered Nurse and a bachelors degree

d. A Bachelors degree in any field with 5 years of experience working with persons with behavioral health diagnoses

e. CASAC Credential

  1. Experience

a. Two years of experience:

i. Providing direct services to people with Serious Mental Illness, developmental disabilities, or substance use disorders;

OR

ii. Linking individuals with Serious Mental Illness, developmental disabilities, or substance use disorders to services essential to successful living in a community setting.

b. A masters degree in a qualifying field may be substituted for one year of Experience.

Must possess a valid drivers license and be insurable. This position is subject to a criminal background clearance.

Pay: $17.00 - $18.00 per hour plus benefits
