Cuyuna Regional Medical Center * Hospitalist Crosby, MN Professional Patient Care Full Time, 12-hour Varied Shift FTE: 1.0 FTE (80 hours per pay period) Weekend Requirements: Occasional weekend shifts Posted 06/18/2024 Req # 13239 Our hospital is looking for an experienced Hospitalist to join our acute care staff. The Hospitalist will admit patients to the hospital, consult on surgical admissions, perform procedures, care for, and discharge patients. Admission process will include history and physical with plan of care and complete orders. Additional responsibilities include rounding, writing daily progress notes with complete plan of care, participating in team meetings, being aware of all patient's treatment plans, documenting findings in patient charts, assessing and managing medications, ordering, reviewing lab tests or other diagnostic tests, and coordination of patient transfers. ESSENTIAL RESPONSIBILITIES Admissions: 1. Responsible for admissions to the hospital and ICU (All pediatric patients age 17 and under will receive care by the Peds/Family medicine physician on call). 2. Admit with history and physical with plan of care and complete orders. 3. Participate in daily multi-disciplinary rounds. 4. Rounding is supported by appropriate documentation. Discharge Planning: 1. Complete discharge of patient by 10 am on day of discharge when possible. 2. Current medications ordered and discussed with appropriate personnel, patient, family and/or caregiver. 3. Provide discharge instructions to the patient and family/caregiver with question/answer time. 4. Completion of discharge summary. Additional Duties: 1. Address all phone calls, concerns, and questions related to hospitalized patients. 2. Respond and aid in management of Code Blue (cardiopulmonary arrest) in any department with collaboration from the E. D. Physician who is also present; Rapid Response in any department, and Code Neuro on the floor or ICU. 3. Triage admissions from the ED and ambulatory services (primary care, oncology, surgical services, etc). 4. Collaborate management of surgical patients with the specific surgical services. 5. Participate in handoff regarding new admissions, updates on patient progress, and needed follow up, with the outgoing hospitalist at shift change. 6. Appropriately triage and divide patients (for admissions and rounding) among oneself and the APPs on members of the Hospitalist team. 7. Cosign APP's inpatient H&Ps and home care face-to-face documentation. 8. Participate in hospital committees and performance improvement activities. Education and Experience: Physician with MD or DO degree; Board eligible or board certified. Special Skills and Aptitudes: Demonstrate excellent communication with primary care physicians and other health care team members to ensure continuity of care post discharge. Works closely with interdisciplinary teams on all units. Maintain confidentiality in all aspects of the job. Excellent written and verbal communication skills. Ability to work independently. Effective time management and problem-solving skills. Knowledge of health care management and the hospital care environment. Experience working in a total quality improvement environment. Demonstrated leadership skills. Working Conditions/Physical Requirements: May be exposed to communicable and infectious diseases. Possible injury from patients during excited phases of illness and cuts from sharp instruments. Possibility of strains due to lifting patients or handling heavy equipment.
