Job Details Job Location Cleveland, OH 44103 - VGS - Cleveland, OH Education Level Bachelor's Degree   Salary Range $16.80 - $17.80 Hourly Travel Percentage Up to 50% Job Shift 8am - 4:30pm Description Location: 2235 E. 55th St. Cleveland, OH 44103 Shift: Monday - Friday, 8:00am - 4:30pm FLSA: Nonexempt Develop and coordinate implementation of vocational rehabilitation programs for participants receiving CBWA assessments, work adjustments, and summer youth work experience program. Work closely with referring counselors to promote services and programs. Work to assure that participant and referral source needs are effectively met and that programs are constantly improving and expanding. Work with referral sources to schedule individuals into assessments. Provide orientation and intake for individuals in assigned services. Assist individuals in the development of individual success plans consisting of targeted behaviors, measurable outcomes, and a realistic vocational goal. Monitor participant's progress toward their goals that are set in the individual success plan. Instruct participants on the development of critical job retention behaviors. Observe and assess participants work behaviors maintaining accurate and up to date case notes and other appropriate documentation. Information may also be provided by Job Coaches assisting onsite. Develop appropriate work accommodations, and assist participants in implementation of these accommodations in the work environment. Write progress and final reports, including recommendations, plans, and implementation. Maintain all necessary case records using web based case management system, including initial paperwork, case notes, timesheets, payroll, and attendance records in an accurate and timely manner and in accordance with CARF, funding source, agency, state and federal records. Coordinate, schedule, and attend staffings for participants with the referral source to provide information regarding consumers' progress toward individual goals. Generate billings and accruals for program participants in a timely and accurate manner. Qualifications Bachelor's degree from four-year college or university; or one to two years experience or training; or equivalent combination of education and experience. Must have reliable transportation to visit employer sites throughout Cuyahoga County and occasionally to neighboring counties.
