Job Summary and Qualifications

  • The Cardiac Patient Coordinator role includes the cardiac quality management and Chest Pain Center Coordinator roles.

  • Cardiac quality management is responsible for conducting chart reviews to ensure compliance with established nationally recognized quality patient outcomes as required by TJC, CMS QIO’s, other state and federal agencies, and NCDR/ACC through concurrent review.

  • The CPCC is responsible for maintaining data at the facility to help guide process improvement and ensuring that ACC guidelines are adhered to by clinicians.

  • The role is responsible for the coordination of the AMI/PCI patient’s care including patient education and follow up.

  • Assisting the CARE ASSURE team in completing follow up phone calls to AMI/PCI/Low risk CP patients. The role is responsible to help facilitate meetings among clinical teams to assist with process improvement and maintaining guidelines.

  • Assure AMI 30-day mortality and readmission tracked with efforts focused on improving outcomes toward the top quartile.

  • CPC Coordinator is also responsible to lead AMI and D2B/Chest Pain Center team meetings in collaboration with Cardiovascular Medical Director to provide feedback and develop care initiatives for the cardiac population.

  • Maintains communications with clinical disciplines directly involved in the care of the patient, and aligns core measure requirements as necessary.

  • Trains and educates nursing and medical staff in the area of data base requirements.

  • Maintains knowledge through self-development, corporate compliance and training, and keeps current on all updates related to cardiac database requirements and best practices.

  • Assists in the planning and implementation of interventions to ensure best practices are implemented for database compliance.

  • Assists in the improvement of the accreditation process by verifying the accuracy of abstracted data, minimizing data calculation and aggregation errors and confirming that the data are as complete as possible.

What qualifications you will need:

  • Must be a graduate from an accredited school of Nursing: Associate's Degree in Nursing or higher is required

  • Must be licensed as a Registered Nurse in the state which you practice and/or hold an active Compact/Multi-State RN license

  • A current American Heart Association (AHA) or American Red Cross Basic Life Support (BLS) Health Care Provider is required

  • A current American Heart Association (AHA) or American Red Cross Advanced Cardiac Life Support (ACLS) Health Care Provider is preferred and required to be obtained within 6 months of hire/transfer date

  • Minimum of 3 years cardiovascular nursing experience required
