Division: Health Reports to: Home Healthcare Manager Worksite: Health Division, site can vary Work Hours: M-F, 8:00am 4:30pm, some evenings and weekends LLBO Status: Non-Exempt Safety Sensitive: Yes License/Certification Requirements: Valid RN State Licensing required MN State Licensing preferred. CPR certification and maintenance, Valid MN Driver's License, current insurance or be insurable. Summary: Responsible for providing medical cares to our community members in their homes, while also working as a liaison in accessing other services our clients may need. Duties/Responub111fies: Visits clients in their homes, spends 75% ofthe position in homes. Provide individualized nursing care to patients. Collaborates with other professionals to plan, implement and evaluate care. Setting up medications, and evaluating client's compliance. Demonstrate competency and use of nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation). Utilizes appropriate age and population standards. Cares for patients in all phases of preventative care, health maintenance, diagnosis, treatment, and follow up as patients move along the continuum of care. Responsible for planning and coordination of care, patient assessment, patient education, triage, and various other nursing interventions. Wound care. International Normalized Ratio (NR), and blood draws in the home. International Normalized Ratio (INR), and blood draws in the home. Vaccinate in the home. Appropriate, accurate and complete medical record documentation Functions within the scope and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure. Utilizes Fleet or Tribal vehicles daily, personal vehicles only allowed with manager or director of nursing approval. Attend and or conduct all mandatory meetings and trainings as required. Maintain confidentiality and professional ethics at all times. Travel in and out of the state on a possibly regular basis. Understanding and sensitivity to Native American culture. Adhere to policies governing staff conduct. All other related duties as assigned.
