Wound, Ostomy, and Continence Nurse 

Role Summary 

Our client is seeking a Wound, Ostomy, and Continence Nurse to join their team. Under the direction and supervision of the Wound and Ostomy Manager, the wound, ostomy, and continence nurse will provide safe, competent, quality care based on nursing theory and research to a designated group of patients. 

 

Key Results Area 

  • Conducts health assessments focused on wound, ostomy, and continence-related care. Performs focused physical assessments, develops a plan of care based on the diagnosis and individual client needs, and recommends treatments as appropriate 

  • Assist with the development, revisions, and evaluations of standards of care 

  • Assess and document patient response to wound care, ostomy, continence, fistula, and complex wound treatment 

  • Works collaboratively with the health care team and makes referrals to other disciplines as indicated 

  • Teach, counsel, and assist individuals and families to assume responsibility for the prevention of complications as they relate to patients with ostomies, wounds, or continence problems 

  • Routinely provide comprehensive assessments of patients at risk for developing pressure injuries; initiating preventative measures 

  • Participate in unit/department quality improvement and hospital quality improvement activities like incidence, prevalence studies, and nursing quality 

  • Provide culturally competent patient, family, and staff education consistent with cultural, ethnic, religious, and other belief systems 

  • Maintain the security and confidentiality of data and information in accordance with patient rights policies 

  • Other tasks as assigned 

Skills & Qualifications 

  • BSN is required 

  • Must have an active NYS Registered Nurse License 

  • Certified Wound Ostomy Nurse or Certified Wound Care Nurse issued by the WOCNCB Certified Wound Specialist issued by ABWM (American Board of Wound Management), or Wound Care Certified Certification issued by NAWCO (National Alliance of Wound Care and Ostomy)  

  • Three to five (3-5) years' experience in healthcare 

  • Able to fully utilize the hospital's electronic record systems, equipment, and other healthcare and billing systems relevant to this position 

  • Proficient in EPIC record systems is necessary
