Guthrie LPN Patient Navigator - Pulmonary in Binghamton, New York

LPN - Patient Navigator - Pulmonary Nodule Program

Position Summary: The Patient Care Coordinator is a LPN/Clinical position that supports Providers as a patient resource specialist offering management of patient care referrals and accessing regional resources. Assists clinical/professional staff by performing duties in caring for patients as directed by Providers, Director or designee. Functions as effective team member in accordance with Guthrie’s values and service standards. Education, License & Cert: High School Diploma or equivalent required. Licensed Practical Nurse in NYS required. CPR Certification required. Experience: Prefer a minimum of at least two‐years nursing experience. Essential Functions: 1. The PCC acts as a liaison between providers and various community agencies, which includes: school professionals, tertiary care centers, social agencies, and specialty providers. 2. Compose letters of medical necessity for patients, 504 plan letters for schools for patients’ IEP and tutoring needs, OT/PT/Speech documentation of necessity for schools. 3. Process insurance prior approvals for complex patients requiring DME equipment and Enteral formulas. 4. Tracking and documentation of Stericycle Hazardous waste logs, Vaccine refrigerator logs, oxygen logs, employee and provider compliance of mandated trainings and monitoring licensing of all clinical staff. 5. Collect and disseminate clinical updates, lab POCT updates to providers and nursing staff. 6. Arrange all medical transportation for out‐of‐county transports for specialist appointments and completing mandated paperwork. NYS Social Security records, multiple agency forms and Family Medical Leave Paperwork required by patients/parents. 7. The PCC works with Child Advocacy Center (CAC), the physician and Family Services with maintaining completed paperwork in its entirety for all sexual assault victims evaluated by the practice providers. Tracks services provided and payment from Crime Victims Board. 8. Oversee the American Proficiency Institute Testing for the practice and clinical staff. 9. Coordinates and oversees hiring and orientation of new nursing staff. 10. The PCC acts as a relief nurse to floor nurses in an emergent situation for staffing purposes as needed. Other Duties: 1. The PCC has to maintain a positive attitude, possess good organizational skills, and be able to muti‐task as providers bring additional tasks throughout the day, and handle parents calls requesting patient needs. 2. ANOS Time & Attendance ‐ Backup Timekeeper.
