Position/Job Title: In Home Support/Supportive Living Provider

Essential Functions and Scope of Responsibilities:
The In Home Support/Supportive Living Provider has the primary responsibility to provide support and assistance, for children or adults, to acquire, retain, and improve the self-help, socialization, and adaptive skills necessary to maximize independence and to live in their private residence.
Job Duties:
Will complete note charting/documentation of recipients progress legibly, accurately, and timely May administer, supervise/assist recipient in tracking medications and healthcare needs Must possess the ability to perform all tasks and activities as needed by this service, as well as exercise sound judgment and provide quality care Possess the ability to communicate effectively with the recipient, family, and your supervisor May be required to manage finances for recipient with proper documentation Must have the ability to work on an individual basis or closely with a team.
